You are hereby required to advice us, on your parcel delivery option by filling in the required form stated above. Please note that the deadline for claiming your cheque/check is exactly one week after the receipt of this email. After this period, your cheque/check will be returned back to the TOSHIBA LOTTERY DEPARTMENT CORPORATION. That is the instruction given to us by the Lottery Corporation Payment Service(L.C.P.S) of the LOTTERY DEPARTMENT CORPORATION. So take note.

On receipt and confirmation of your mail and the required information, we will send you payment invoice which you are to follow in effecting payment to us base on the delivery option convenient for you so as to prompt the delivery of your parcel.
