I went to one of those Brite Smile / Dazzle White sites like many of you who complain did. Then, I filled out the information, but did not like the fact that they did not provide information as to the shipping charges.
I did NOT agree to their terms of service, but the site did not have a "decline" feature. After trying to back out of that page, I turned the computer off since I had not submitted anything to them for approval.
Then I started getting billed. The first time, my credit card company and I sat down and had a three way call with these people. They agreed to remove my name and not bill me any more. The next month, two more charges appeared.
Brite Smile / Dazzle White... whatever they want to call themselves just keep billing you after you've supposedly been removed from their lists. They simply call their company something different each time they bill you. I kept going through the credit card company with their half educated crews and companies kept telling me that it was a mistake, they would take me off their lists, etc. But it dawned on me. The merchants have a number they bill your credit card from. So, I investigated. ALL the unknown charges on my credit card came from ONE company under many different names.
When I call these companies, they deny they are one and the same; all of them provide FALSE information as to where the corporate office is; NONE of them would tell me what product or service I allegedly got from them where I owed for it. So, if Brite Smile does not own EZ Pak (one of their many aliases), and if EZ Pak bills me and cannot tell me what for, they have committed a crime.
