I had Dr. Collins as my Dentist for 6 months, And I had all my dental records including my x- rays of my mouth done in his office. Since then, he has closed down his dental practice I.E. "Terminated" from dentistry, I would like my Wifes and Mine dental records available to me, so we can continue our dental care some where else.
