I applied for change of address(CARD NO: 4718 6100 0078 5031) for correspondence from

to my permanent address of

AJAY KUMAR SINGH
138/116, TELIAR GANJ

ALLAHABAD(UP)-211004

for which I attached all the relevant documents i.e. telephone bill/my PAN card/Driving Licence.

All these documents I couriered in Nov'08(at address mentioned in the credit card); and since I have shifted from that place to abroad and have paid around INR55, 000 of amount as well but till date the changes have not been made. Also if I can get the statement at my e-mail ID [email protected] then also I can continiously use it.

Also, at the contact no. mentioned in the credit card i.e. 022-25274407 no one bothers to pick the phone.

Hence forth I request for neccesary changes or some one has to contact me at 0094778009400.
