On 12/8/2009 a company by the name of "FormsLeader" authorized a $4.65 cent charge on my Credit Card.
I had never heard of this company in my life before. Turns out someone stole my Credit Card information and used it online to order some legal forms.
I've contacted the merchant and they only provided me the IP address and the email used for the transaction made on my stolen CC information.
The company has no address they only provide a phone number/email for customer support. I'm letting the world know that the merchant has NO form validation on the order form for their services they offer online. Therefore stolen Credit Cards can be used on this merchants online order form and there is no validation!
I took screenshots of me ordering on the order form (see attached) with bogus information and a bogus credit card, and the page does no validation against CC info name/zipcode therefore the transaction is successful.
The person that stole my Card may be doing this to other people. A police report will be filed and I plan on possibly suing the company for not having form validation on a secure SSL page. I've also filed a compliant with BBB
This is the only information I know about this "company"
