unauthorized charges to my cc to reservation rewards

after I ordered pictures from shuttlerfly they gave my cc info to reservation rewards who charged me $12 per month for 8 months which I never authorized. I am requesting
a refund for the amount that was charged to my account.

moned deducted from my account without permission

When I ordered pictured from shuttlefly $12.00 was deducted from my debit account from Septemper 2008 thru February 2009, then again form April 2009 until May 2009 each month. I had to change my debit card number on my account to prevent
the rewards program from deducting any more money from my account. And when
i called them to stop the deductions they stopped until I ordered pictures from
shuttlefly again then they started deducting the $12.00 each month again.
